Job Details

• Candidates with an AZ home address on their drivers license will not be considered - please include a copy in submission
• Banner has a specific cover page! PLEASE vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in.
• Start Date should be on or very quickly after the Order Start Date listed.
• All RTO should be listed on the cover page, must be less than 7 days.
• RTO will not be approved in the first week of assignment or holidays.
• All facilities the traveler is open to working in should be listed on the submission.
• If interested in 'any' location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
• All shifts the traveler is open to working on should be listed on the submission.
• All departments the traveler is open to working must be checked on the cover page.
• All candidates submitted should be preclosed and ready to accept.

Job Requirements

• 24 months minimum IR experience for Level 1 Trauma and Stroke procedures - REQUIRED
• 24 months minimum of current Special Procedures Technologist (Angiography) experience - REQUIRED
• Experience setting up/scrubbing into sterile field, experience with angio, filters, ports, caths, tubes/drains - REQUIRED
• Certifications must be American Heart Association OR American Red Cross, active upon submission and INCLUDED IN SUBMISSION - REQUIRED
• No License Hits - REQUIRED

Schedule Information

• Banner will not approve RTO within the first week or holidays. Can not exceed 7 days.

Additional Details

• Flu vaccination - Banner declinations can be applied for (reach out for forms)
• Please add vaccination status in the submission notes
• BANNER HAS VERY SPECIFIC BACKGROUND REQUIREMENTS THAT MUST BE ADHERED AND ATTESTED TO. PLEASE LOOK AT THE COMPLIANCE REQUIREMENTS IMMEDIATELY
• Vascular Interventions, Device Implants, Central Line Placement, Stroke Thombectomies, Neuro Interventions experience - Preferred
• Travel experience - Preferred
• Banner Cover Page completely filled out
• Resume with all facilities, facility size, beds, trauma level, and units listed
• Skills Checklist specific to the discipline (must be less than a year old)
• Active License (verification must be less than 30 days old)
• Certifications (must be active at time of submission)
• Driver's License
• Banner Assesses a 2% payment discount fee and a 2% technology fee on all transactions.
