Job Requirements

• BLS
• Proof of vaccine Sept 2024 booster, declination or exemption (with agency letterhead)
• Attached scanned proof of 2 vaccinations with dates or primary source IgG titer with lab print-out to candidate's record in Trinity VMS
• Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later. Hep B titers will NOT be accepted. Proof of vaccination must be uploaded into Trinity VMS
• Proof of vaccine, declination or exemption (with agency letterhead). Declination not accepted for CA or IL. Flu dates: 10/1/2024-4/30/2025
• Attached scanned proof of 1 vaccination with date or primary source IgG titer with lab print-out to candidate's record in Trinity VMS
• Upload a current, non-expired TDAP vaccine. Declinations are accepted if the assignment does not require Pediatric interaction. Agency form acceptable for declination. Declinations not allowed in Michigan
• Provide documentation from an exam within the last year that candidate is fit to work. Attach scanned copy of certification to candidate's record in Trinity VMS
• 9 Panel completed within 30 days of start date, preferred within 14 days of start. Must be completed and resulted before start with MRO signature. Must include the following: Cocaine, Amphetamines, Opiates, 6- Monoacetylmorphine, Propoxyphene, PCP, Barbiturates, Benzodiazepines, Methaqualone, and Methadone. (NO THC)
• Resume, Skills Checklist within a year, 2 Professional References
• All engaged contractors must have page 27 signed form on file in Trinity VMS
• Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement. All engaged contractors must have page 5 signed form on file in TrinityVMS
• All engaged contractors must have page 1 signed form on file in TrinityVMS
• Federal, Nationwide, County Criminal from last 7 years. Also include SSN Verification. Must be dated within 30 days of start. Check all addresses for primary name and AKA names. (Maryland Counties will be included within State Search)
• Attach completed 3rd Party Screening Company report of candidates highest level of academic education (degree - if no degree High School Diploma or GED must be verified) into candidate's record in Trinity VMS. *If job description requires specific certification(s), then certs will need to be provided at onboarding
• Attach scanned EPLS report from website ( or result provided by third party screening company dated within past 30 days into candidate's record in Trinity VMS. All Aliases must be searched
• Attach scanned OIG result print-out from the website () or result provided by third party screening company and dated within past 30 days into candidate's record in Trinity VMS. All Aliases must be searched
• Must be dated within 30 days of start. Check all names used and locations. Attach scanned report to candidate's record in Trinity VMS. All Aliases must be searched. Please use MI ONLY: Must also include statewide
• Must be completed and uploaded as soon as possible
• 2 Step PPD OR IGRA (Quantiferon/Tspot)
