You’ll be responsible for completing the Resident Assessment Instrument and coordinating services among various stakeholders. Your Responsibilities

• Complete the Resident Assessment Instrument in accordance with federal and state regulations and company policies.
• Act as an in-house case manager, coordinating care with physicians, families, third-party payers, and facility staff.
• A detailed job description, including all responsibilities and physical requirements, will be provided during the interview process upon request.

Qualifications

• Licensed Registered Nurse with a current, active license in the state of practice.
• Current CPR certification is a must.
• Minimum of two (2) years of clinical experience in a healthcare setting.
• At least one (1) year of experience in a long-term care setting.
• Prior experience as an MDS Coordinator with demonstrated expertise.
• Ability to maintain regular attendance.
